There are 3 Altars to discover in the Westering Lowlands:
Altar #34 can be found a bit behind the road leading to the Fractured Peaks.
Altar #3 can be found near the event. It's in the camp.
Altar #33 is between the events, opposite of Smuggler's Underpass cellar.
Deep Forest

There are 3 Altars in the Deep Forest area:
Altar #32 can be found in the abandoned camp right after you leave the narrow road from the Westering Lowlands.
Altar #2 is located near the event in the southern border of Scosglen, in a small cave near the Dry Steppes.
Altar #28 can be found at the end of the road near the strait (edge of the map) connecting Frozen Sea with Twin Seas. It stands on the edge of a hill near an old stone wall.
Tur Durla

Tur Durla Stronghold has one Altar.
Altar #29 is easy to miss, as you have to jump over the precipice from the main road leading to the center of the Tur Durla Stronghold.
Northshore

There are 4 Altars to discover in the Northshore area.
Altar #27 can be found after exiting the narrow road from the Deep Forest. The Forgotten Shack cellar is nearby.
Altar #26 is hidden in an abandoned wooden structure in the center of Northshore. Calibel's Mine dungeon is nearby.
Altar #23 is in the northern part of the map, right above the Frozen Sea. It is located next to a large rock.
Altar #24 is also in the northern part of the map above the Frozen Sea, in a dead end between the rocks.
Hope's Light

Hope's Light has one Altar.
To get to Altar #25, you have to jump over several chasms that are on the road leading to the center of Hope's Light Stronghold. You don't have to recapture the entire Stronghold to get there.

Altar #1 is located in the northern part of the map, near the Apex of Misery main quest.
Altar #22 can be found near the event and the Penitent Cairns dungeon.
Altar #21 can be found in a small dead end right next to the passage to the Raethwind Wilds dungeon.

Altar #30 can be found to the right of the exit from the narrow path leading from the Northshore area. It is hidden among the trees.
Altar #31 can be found near the road leading to the Westering Lowlands. It's easy to spot - it's near a big chasm.
Altar #16 can be found near the road leading to The Downs. It is hidden among the trees.
Strand

Altar #20 is located between the Domhainne Tunnels and Luban's Rest dungeons. It is inside an old wooden hut.
Altar #19 is located near the world boss arena. It's hidden in the ruins.
To reach Altar #11, you have to climb the hill. Nearby is the Drowned Burrow cellar.

Altar #12 can be found near the road leading to The Downs - in the western part of the Highland Wilds.
Altar #5 is located near the road leading to the Fractured Peaks. To get to it, you have to climb the cliff.
