There are 6 Altars to discover in Scouring Sands.
Altar #6 is in the northern part of Scouring Sands. It is at the very end of the hill in the ruins. There is an event nearby.
Altar #8 can be found near Tarsarak, where you can unlock a fast travel point. It can be found in the ruins of an old building.
Altar #7 is near the Halls of the Damned dungeon. You will find it under a small rocky hill.
Altar #21 is in the northern part of Scouring Sands. It is partially buried in the sand and tilted to one side.
Altar #9 is right next to the entrance to the Abandoned Mineworks dungeon in the part of the map marked in red. To get to it, you have to get through the platform section.
Altar #11 can be found near the Forlorn Cavern. It is located by the rocks, on the edge.
Alcarnus

Alcarnus Keep contains one Altar.
Altar #5 is in the north-eastern part of the Alcarnus Stronghold. It is hidden under wooden beams.

Altar #12 can be found near the Renegade's Retreat dungeon. It is located on the edge by the destroyed bridge.
Altar #22 is located in a dead end in the western part of Caldeum. This location will be unlocked at the beginning of Act VI.
Altar #24 can be found in the southern part of Caldeum. It is hidden under wooden beams. This location will be unlocked at the beginning of Act VI.

Altar #10 can be found on the path connecting Amber Sands with Scouring Sands. It's in the recess.
Altar #4 is in the eastern part of Amber Sands. It is located in the ruins covered with sand.
Altar #1 can be found near the northern border of Kehjistan in the ruins of an old building. You can also find an event in this area.
Altar #3 is in the central part of Amber Sands. It is leaned against rocks.
Altar #14 can be found near the Sunken Library dungeon between the rocks.
Altar #2 is under the palm trees in the western part of Amber Sands by the water.
Altar #19 is at the end of a long path that starts in Ragged Coastline.

Altar #20 is in the eastern part of the Altar of Ruin Stronghold, right next to the big red pit.

There are 4 Altars to discover in the Coastline.
Altar #18 can be found by the water, at the edge of the western part of the map. To get to it, follow the narrow path between the rocks.
Altar #15 is east of the Uldur's Cave dungeon. It hides behind the leaves of trees.
Altar #17 is at the edge of the map, in the western part of the Ragged Coastline. You will find it in the corner, among the rocks.
To get to Altar #16 you have to climb into a small cave. An event is also nearby.

Altar #23 can be found near the Wicked Sanctum cellar in the northern part of Southern Expanse. Around it you will see destroyed trees (without leaves).
Altar #25 is located near the village of Gea Kul. It's in an abandoned camp.
Altar #26 is near the event, in the central part of Southern Expanse. It's hidden under palm trees.
Altar #29 can be found near the Maruder's Hideaway cellar under the palm trees.
Altar #28 is by the oasis west of Omath's Redoubt Stronghold.
Omath's Redoubt

Omath's Redoubt Stronghold contains one Altar.
To get to the Altar #27 you have to crawl under the rubble to a small room in the building in the northern part of the stronghold.
