The Spanish Document (1) is lying on the ground right next to the fountain in the center of the town.

Mission of Saint John

Document (2) is on the table in a dim spot. There are a few NPCs nearby.

The Miracle Club

The flyer (3) is lying on the ground in a visible place, near the candles and nuns.

Damage Repair

The Official Document (4) is on the barrel in a dim spot, in the room with nuns and kids.

On This Spot

Document (5) is on the table in one of the run-down bindings.

Journal (6) is in a dim spot. Right next to it, there's also a backpack.

The journal (7) is lying on the boards right next to the Riverside camp, where Lara meets her friend Abby.

Journal (8) is on the ground in a dim spot, on the ground floor in the library.

Journal (9) is in a dim spot, on the ground floor in the library.

De La Cruz Leaves

Journal (10) can be found on the ground floor of the library. Lara has to use the axe to break the cracked wall. The secret is behind the wall.

Journal 11 is on the first floor of the library. You have to break the cracked wall shown in the picture above. This will reveal the collectible.

John the Apostle

Document 12 is on the first floor of the library. It's lying on the ground next to the bookshelves.

Repelling the Demons

The Sketch (13) is on the first floor of the library, next to a big hole in the floor (you have to walk around it).

De La Cruz Leaves

Journal (14) is on the ground in a room away from the stairs on the first floor of the library.

Ranting

The Poem (15) is in the last room on the first floor of the library, near the stained window.

Missionary Work

Journal (1) is in the small area, where the conquistador chest also is located. Lara has to start in the nearby large water reservoir, dive and find a hole underwater through which you can go to the side location.

Last Words

The Note (2) is lying on the ground in a visible place, on the path to the Thirsty Gods tombs.

Testament of T. Serrano

Document (3) is in a big room, where the Thirsty Gods tomb puzzle can be solved. You will find it on the ground next to the mechanism used to regulate the water level.

Alux

Document (4) is in a big room, where the Thirsty Gods tomb puzzle can be solved. You have to solve a riddle with changing the water level and swim into a large structure. You will find the secret after leaving the water, on the way to the chapel allowing you to complete the tomb.

The Hills Are Alive

Document (5) is in a dimly-lit corridor near the entrance to the Tree of Life tomb.

Chechen and Chaca Trees

Document (6) can be found tight after entering the Tree of Life tomb. It is in a poorly lit place next to the Mountain Catacombs camp.

World Tree

Document (7) can be found during the exploration of the Tree of Life tomb when you're setting the platforms into motion.

The Chosen One

Journal (8) can be found while exploring the crypt in the Star Crossed side quest. You will find the secret right after using the crypt opening mechanism and entering it.

Believer's Journal

Journal (9) can be found while exploring the crypt during the Na Przekór side quest - after finding Isabel. Start near Isabella and squeeze through a small hole near the ground into an adjacent area. Examine it. In the place shown in the picture, Lara must avoid the trap. The secret is located just behind her.

Hubris

Journal (10) can be found while exploring the crypt during the Na Przekór side quest - after finding Isabel. Start near Isabella and squeeze through a small hole near the ground into an adjacent area. Now squeeze through the gap between the walls. You will find the skeleton and the collectible right next to it.

Pure of Heart

Journal (11) can be found while exploring the crypt during the Na Przekór side quest - as you're going back to the surface after finding Isabel. You will see the secret right after using the axe with the rope on one of the hooks.

Demons

Journal (12) can be found while exploring the crypt during the Na Przekór side quest - as you're going back to the surface after finding Isabel. You must reach the interactive mechanism shown in the picture above. Investigate the area behind the mechanism selected in the picture. That's where the secret is.
