This page of the secrets guide for Uncharted 4: A Thief's End has locations of the treasures in chapter 12 - At Sea. Our tips will help you complete chapter 12 in 100%.
The six out of all the treasures listed below can be found as a result of the optional exploration of the archipelago. When it comes to the remaining seven, you can find them while searching for the pirate hideout and after you have found it.

Treasure 1 - Combination Lock Tobacco Box
The first treasure has been hidden on the left isle of the archipelago (the larger one). You can find it at the very beginning of the chapter. Reach the isle and steer around it on the left. Stop the boat after you reach the beach fragment shown in the above screenshot. Disembark.
Explore the small grassland. You have to find the smashed crate by the bushes.
Treasure 2 - Brass Pocketed Sundial
You can find the second treasure also at the beginning of the chapter. You have to steer the boat towards the small isle with a ship wreck (the above screenshot). You can find it to the left of the main (largest) isle.
Steer the boat towards the ship wreck and dive into the water. You have to find a hole in the ship. Swim into the ship and find the treasure shown in the above screenshot (upper deck).
Treasure 3 - Silver and Wood Tankard
The third treasure is yet another one that you can obtain right after you start this chapter. In this case, you have to steer the boat towards the right main isle of the archipelago (the smaller one with a tower on it). Make it over to the main grassland, where there is a skeleton, among others. The treasure is by the rocks shown in the above screenshot.
Treasure 4 - Persian Silver Pen Box
You find the fourth treasure right after you find the one above. Also this one is on the large isle on the right. In this case, you have to take a look around the beach behind the cave entrance shown in the above screenshot. This is where you find the item.
Treasure 5 - Panamanian Pendant
The fifth treasure is yet another one that you can find at the beginning of the chapter. Just like in the case of the two above collectibles, you have to search for it on the large isle on the right. Steer around the isle to find the small beach fragment shown in the above screenshot.
Once there, get to the land and find the treasure in the spot shown in the above screenshot.
Treasure 6 - Persian Leather Notebook Case
The sixth treasure is the last one that you find at the beginning of the chapter. In this case, you gave reach the back of the left (larger) main isle. Continue on your boat until you find the ledges shown in the above screenshot.
Stop the boat right underneath, climb onto the roof of the boat and make it over to the top ledge. The collectible is on the top ledge shown in the above screenshot.
Treasure 7 - Deccani Bronze Spouted Bowl
The seventh treasure is the first one that you find after you start searching for the pirate hideout. You obtain it after you approach the ruins shown in the above screenshot. You reach this location after you follow the large compass-needle-like arrows, after the climbing in the cave.
Examine the area to the right of the ruins. The collectible is in the spot shown in the above screenshot.
Treasure 8 - Mughal Elephant Carving
You find the eighth collectible after Sam walks over the bridge that collapses under his weight. You will have to split for a moment there. Start by climbing onto the right fragment of the snapped bridge shown in the above screenshot.
After you reach the upper ledge, head towards the area on the right shown in the above screenshot. There, in a dimly lit spot, you find the treasure.
Treasure 9 - Ottoman Helmet
This treasure is one of the easiest to locate. It is inside the tower shown in the above screenshot. You cannot miss it, because Nate will have to climb to the top of it later into the chapter.
Treasure 10 - Deccani Bronze Vase
You can obtain this treasure soon after you find the previous one. You have to reach the very top of the overgrown tower. There is a grappling point, shown in the above screenshot, there.
Use grappling hook on the grappling point, BUT take just a slight swing that will allow you to reach the neighboring ledge, AHEAD OF the steep surface (if you start sliding down, return quickly to the previous checkpoint). Ignore the escarpment and reach the ledge on the left shown in the above screenshot.
Treasure 11 - Strange Relic
You can find the eleventh treasure soon after you have found the previous one. This time, slide down the escarpment to a lower level. Then, turn left and take the path among rocks shown in the above screenshot.
Take the ledges on the left and reach the top ledge. Turn towards the ledge in the distance shown in the above screenshot. You have to jump towards it and use grappling hook in mid-air on the grappling point above. Take a swing, reach the ledge and collect the item.
Treasure 12 - Hamsa Mythical Goose
You find this treasure right after you have reached the pirate hideout that you can recognize by the large statues. Use grappling hook on the spear of one of the statues. After you reach the grappling point, turn on the rope towards another spear, jump towards it and use grappling hook again. You have to reach the small ledge by one of the statues shown in the above screenshot. You have to get there before you jump down.
Treasure 13 - Pique Tortoise Shell Box
You can find the last treasure while exploring the ruins in the pirate hideout. You have to do this BEFORE you go to the chamber with the second puzzle. First of all, reach the flooded corridor shown in the above screenshot.
Jump into the water and start swimming ahead. Keep an eye out for ledges that you can climb onto. This is where you find the collectible shown in the above screenshot.
