Mottled Purple Gourd is not found in the form of loot. It needs to be purchased, and it becomes possible only in the further stage of the game. Get to the Ashina Depths and explore it until you reach the sculptor idol called the Mibu Village. Next to the statue you will find Exiled Memorial Mob.

You can place the Mottled Purple Gourd on the skill bar panel - Mottled Purple Gourd | Unique items in Sekiro Shadows Die Twice - Unique items - Sekiro Guide and Walkthrough

You can place the Mottled Purple Gourd on the skill bar panel. Its action allows you to reduce the negative state Terror that may appear on the screen in the form of a purple stripe. The Terror bar is filled primarily by taking damage from various types of ghosts, as well as from selected bosses (for example Headless Guardian Ape). The Terror damage can gradually decrease health points of the hero. Complete Terror bar means, unfortunately, instant death.

Taking a Mottled Purple Gourd nullifies the current bar of Terror and also partially calms the hero down for re-increasing it for about one minute. In case of long fights you may need to re-use Mottled Purple Gourd, but the hero, fortunately, has five stacks of it. The stock of Mottled Purple Gourd is automatically updated after using the rest options on any sculptor idol. So it is better than typical sedative, which is a single use item.
