Reach the bunker
You do not need to get prepared for the last mission, in any definite way. Still, if you want to, you can craft, or purchase, some Jam Coms beforehand and unlock new skills connected with driving and hacking into the environments. To start the only mission of the fifth act, you need to return to the bunker in The Loop. See the cutscene here, which features T-Bone.
Gain access to the main ctOS building
Your new destination is the ctOS headquarters in the Mad Mile. You should not hurry too much with reaching your destination. The hacker will be intervening with the elements of the surroundings and you need to watch out, especially, for the exploding pipelines that you need to drive around, while keeping a safe distance.
After you approach the ctOS HQ, get out of the car and cover the rest of the distance on foot. Try hacking into the building's system. After the minigame starts, modify any of the elements of the puzzle, thanks to which you will find out that the firewall is, as of now, too strong. Listen to the conversation with T-Bone, who will suggest what to do next.
Upload T-Bone's virus
Your next task is to upload the virus to the ctOS system, and you need to do that in three different spots, spread all around the city. You can visit these points in any order you wish to, although I recommend that you follow the instructions provided in this guide. I also recommend that you add these destinations as the destinations in your GPS because, during the mission, Damien will be trying to lead you astray with fake markers (this will be especially important while you will be trying to reach the third spot). The first spot is on an islet, to the west of the Mad Mile (the above screenshot).
Along your drive to the first spot, where you are supposed to upload the virus, you need to watch out for the elements of the environments that Damien hacks into (pipelines, bollards, road spikes, drawbridges, etc.). However, this is not the only problem that you have. Soon, the fun will be joined by the police and they will start scanning the area. If this is an option, try to prevent them from locating Aiden, by using the Jam Coms.
If you fail to avoid the police scans, unfortunately you will have to deal with a police pursuit and you need to keep in mind that they will be considerably obstructing you from reaching the destination. Things will get additionally complicated after it turns out that the majority of the elements in the surroundings are, exclusively, to be hacked by Damien. While escaping the police, you therefore need to rely, mainly, on flawless driving, using side roads or driving through garages. Also, do not bother if you need to change your means of transport, but do that only if it is relatively safe around, because the pursuit may also be reinforced with police choppers.
After you approach the first destination point, try to block the entrance to the square, with the car, or use hacking to extend Spikes from the ground. Start hacking into the interactive point. The important piece of information is that you cannot walk away too much, while uploading the virus. Therefore, you can move around only to a limited extent and, if you do not want to kill the policemen, try only to distract them (if only they arrived here, while following you).
The second location to upload the virus is in the central part of The Loop. The location itself has been presented in the above screenshot and it is the same location, where you have hacked into the ctOS control center, in the third mission of the first act.
Just like earlier, you need to try to avoid the police pursuit or (if you fail to do that) try to avoid collisions with the other vehicles and indestructible elements of the surroundings. I recommend that you drive into the square that neighbors the interactive point and lock the gate (hacking), because this will slow the police down. Also do not forget not to walk away from the interactive point, after you start uploading the virus.
The last, third, location to upload the virus is in the ctOS control center that you have visited earlier in the Eastern part of the Mad Mile (the above screenshot). It is necessary that you set this location as your destination point, on the map, because Damien will be trying to confuse you, when it comes to the point's actual location.
Again, I remind you about the necessity to keep your vehicle in good condition. After you reach the pier, drive into the empty base and lock the gate. Wait for the virus to be uploaded for the last time.
Lose the pursuit
If the police are currently chasing Aiden (which is highly possible) you now need to lose them. The good news is that you can hack into the elements in the surroundings again and, thanks to raising the drawbridges, extending bollards or blowing up the pipelines, escaping the police cars should be easy. Drive outside of the grey area to end the pursuit.
Disable ctOS
Once again, you need to visit the ctOS headquarters in the Mad Mile. Walk into the building and see the message from DedSec, who Aiden does not want to help gain temporary access to the system. Approach the marked terminal, hack in (hold down the appropriate key/button) and complete the hacking minigame.
After you load the view from the server room, switch to the neighboring camera and hack into the new router. The last hacking minigame is a pure formality, because you simply need to unlock the exit point. Finally, hold down the hacking key/button to disable the ctOS.
Reach Damien's staying place on the boat
Head towards the pier to the East of your current location (it is a good idea to set the lighthouse as your current destination). After you get there, steal one of the boats moored here.
You need to reach the lighthouse, in the boat. After you get there, exit the boat, e.g. next to the small pier. Hack into the interactive elevator here and, after you enter the lighthouse, start climbing up the stairs and the ladders.
The Grand Finale
After you get to the very top, you will see a cutscene of the confrontation with Damien and Jordie. Towards the end, you will have to take several simple steps - you need to activate Aiden's smartphone, (hold down the hacking key/button), toss Jordie over the balustrade and take a shot at Damien.
Reward for completing the mission: notoriety, cash + experience points for losing the police pursuit
