On this page of our The Medium game guide, you'll find a description of stage 4 of the game, Meeting with Sadness. Here we have outlined all the interactions you need to have with Sadness. Your main task is to reach the upper floor and find the girl. However, you can perform some minor interactions that will help you understand the plot and increase the immersion.
First Meeting with Sadness
Once you enter the hotel, you are free to look around it. Here, you will find one postcard. You can check the locations of collectibles on a separate page.
After checking the entire floor, approach the locked hallway. When you try to get to the bars, you will hear something constantly hitting a bell.
Go to the bell and try to use it. You will be distracted by a ball falling down the stairs. This will summon your spiritual part – you will encounter Sadness.
Sadness will talk to you for a while and then run away. You won't meet her until you reach the second floor.
The stairs, however, are blocked. You must use the elevator to get to the higher floor.
Use the appropriate button and wait to see what happens next. Remember that whatever happens, you are not able to change it. All events are part of the story line.
Elevator repair
When you get to the floor, you will not be able to leave the elevator. Switch to your spirit form and exit the elevator.
Remember that you can't move around in this state for too long, as your normal form will pull you back. When that happens, you will have to start exploring the hotel all over again.
Go down the stairs and head to the tombs located at the end of the living room. Absorb the energy that will enable you to perform spiritual attacks.
Go back upstairs, get closer to the elevator and use the spirit blast. This will open the elevator doors and allow your normal form to leave.
Searching for the razor
When you reach the hallway, your spiritual version will be attacked by moths. You can protect yourself from them by using the spirit shield.
There is an achievement associated with this - Medium Rare.

Use the door on the left. Switch to your spirit form and run over the threshold. In the room, you will find only one interesting thing - a key.
The strange key is on the small table on the right side of the room. You will need it in a moment, so put it in your inventory.
Exit into the hallway and head to the door on the right, near the evacuation sign. Use the key and then enter the room.
You can immediately jump down to the lower floor through the hole in the floor.
Once you enter the next room, you can immediately examine the vase. The heroine will point out that there aren't any flowers. This will bring back a memory, which we described in detail on a separate page of our guide.
Go out to the balcony – if you move with your regular form, part of the area will collapse making you unable to proceed. Use your spirit form and move to the next room.
After entering the room, head straight to the door with the hole. You can take a peek at what's on the other side, although something might scare you. After watching a short cut-scene, go inside.
Once in the bathroom, approach the tub and drain the liquid that is in it. At the bottom you will find a razor, which will be very useful to you - without it, you will not be able to proceed with the story.
Doing this will unlock Edge of Sanity achievement.

Return to your normal form and approach the wall covered by skin. Cut it open with the razor to get to the next room.
Remember that cutting the skin will open a passage for you in both the normal and spiritual worlds.
Getting to Sadness
When you reach the room, you will see a dried sunflower. Take it out of the pot – it will be useful soon.
Return with the sunflower to the room with the hole in the ceiling and place the flower in the vase.
After that, you can gain the energy you need to move on. If you don't recharge at this point, you will have to go back – you won't be able to make a spirit strike nor a spirit shield.
Return to the corridor and head to the very end, to the stairwell passage. Your spirit form will be attacked by moths – you must protect yourself using a spirit shield.
Go to the upper floor to get to the next part of the building.
At the top, you must move the cart that blocks the passage. You will do it without any problem, you just have to push the object so that the door is exposed.
After exiting the stairwell, turn right. You must cut the skin using the razor to get to the next part of the room.
You will arrive at Thomas's office. When you try to open the door, Sadness will appear – Marianne will talk with her.
