You will find the charm in Hunter's Lodge at the very beginning of the game. It's lying on the bed.
Officer's Badge

This log is in Quarry in the El Gigante cage. You can only access the cage after defeating El Gigante in Chapter 4.
Broken Cell Phone

The phone is on the first floor of the Church, which is visited after completing the Dials puzzle next to the stained glass window. You can get it either in Chapter 4 (before meeting Ashley) or in Chapter 5 (having Ashely along).

A secret obtained in the Village Chief's Manor attic. Getting it means returning to the Manor in Chapter 5. In the first floor room, use the switch, and an attic hatch will open. Ashley can climb up and deploy the Collapsible Ladder. This will allow Leon to access the attic.
Look for the photo on one of the pieces of furniture.
