First off, you have to turn off the lights. This is an important step that will make the whole puzzle easier to solve.
The next step is to head to the bedroom. You will find many items there, such as balls, blocks, cans, etc. You need them to solve the puzzle.
Pick up one of the toys (whichever one), Six will also take one, although she will not make any other effort to solve this puzzle.
With the lights off, see the rays coming from the next room. You need to throw the toy straight at the image so that it allows you to move on.
You will end up in a small room. To get out of it you simply have to jump over the trapdoor in the middle of the room. It is very unstable, so it will open when you make a few accurate jumps.
Your next move should be to run through the narrow tunnel. You will end up in another small room. Go all the way down to open the passage further down.
Move closer to the ventilation shaft. Grab the grate and start pulling on it. Six will help you get out of this part of the orphanage.
