Unlocking the quest
You can start this mission after completing chapter 2 - after completing objectives in the Hirokawa Shrine. A quest marker should automatically appear on the map.
The quest giver is the Patient, whom you will meet next to the hospital building.
Search for the girl on the 4 floor
You have to go back to the hospital building that you visited at the very beginning of the game. Use the front door.
On the ground floor of the hospital, you can encounter Visitors, but it is possible to use stealth attack on them. Ultimately, you have to reach the elevator and use it to travel to floor 4F.
Start exploring the patient room on the fourth floor. In room 403, you will meet the spirit of a girl with whom you need to talk.
Head to the nurse station on the 3rd floor
There is no elevator access to the third floor of the hospital building. Go further through the main corridors, get rid of the Visitors along the way and use the stairs shown in the picture to descend to the lower level.
The surrounding corridors are patrolled by Visitors - you can take them down quietly or defeat them in battle.
Search for documents
After reaching the nurses' room, you have to locate the Kent Insect - Fatal Accident Report shown in the above picture. They are on one of the shelves.
Open the sealed door
You have to reach the door marked by the game. By reading the document, the main character will know that the combination for the door is 74398. Choose the right option to enter the code.
Search the room
Follow linear corridors to the destination. You reach a place where you will have to talk to Owada's spirit. You will learn from the ghost about the Visitors plaguing this place.
Defeat the monster
The monster - an elite Visitor - will appear shortly after entering the new corridor. You have to defeat Kuchisake - this enemy uses large scissors for melee attacks and can charge at you. You can stop a charging Kuchisake by using Fire Weaving. Try to avoid close combat and run away from the enemy after it gets too close to you.
Fight until the monster's core is exposed and pull it out to end the fight. The only thing left for you to do is reach the place where the spirits are and listen to their conversation.
