Where's Blood-Stained Shrine?
The Blood-Stained Shrine is located in the western part of Iki Island. You can reach it without any problems.
Solving the puzzle
At the base of the statue, you will find a scroll - it contains a clue to the puzzle's solution.
You must follow these steps:
1) Wear Kensei Armour - You get it during the main campaign, i.e. in the basic content of the game. Armor is the reward for doing The Six Blades of Kojiro mythic tale. If you have not dealt with this quest yet, go back to Tsushima and complete that tale.
2) Getting injured - The easiest way to achieve this is to fall from a high spot near the chapel. Climb the surrounding rocks and jump from the highest point towards the shrine.
If you have done both steps correctly, you will see a cut-scene.
Rewards for solving the Blood-Stained Shrine puzzle
Blood-Stained Shrine offers rewards that are related to Bloodborne. These are:
1) Yharnam Helm
2) Yharnam Vestments - This is not really a brand new armor, just one of the variants of the Kensei Armour.
