One of Chakra Materia can be obtained after reaching Sector 7 Slums, after meeting Avalanche group members. To get it, you need to travel to the abandoned factory.

Chakra Materia #2 is in the location shown in the screenshot above. Simply pick it up so that it finds its way to your equipment.
