Go down twice and use the hook to reach a rock shelf on which you will find a med kit and a kevlar vest. From there, go up (use the hook) and you will enter a small cave. Inside, you will find the letter near a large blue face.

You can enter the cave through a small hole in the ground (use the hook). Swim to a small chamber. Behind the fire, near the statue, you will find a bag with a letter.
