This page of the Diablo 2 Resurrected guide contains information on how to start and complete the Fallen Angel quest in Act 4.

The Fallen Angel quest will start in Act 4 when you talk to Tyrael. You must deal with Izual, the Fallen Angel. You'll find him in the Plains of Despair, outside the Outer Steppes.

After entering the Plains of Despair, move slowly and carefully. Izual can be found more or less in the middle of the location. He is a much stronger enemy than Mephisto and that's due to his cold attacks (to which it also has increased resistance itself) and a very large amount of health. Be sure to eliminate all his helpers first. After completing the quest, quickly return to Tyrael, because he has a reward for you: 2 additional skill points to allocate.
